Our providers collaborate across specialties to evaluate vestibular, visual, cognitive, cervical, and physical contributors affecting recovery.
We partner with referring providers to support individualized treatment planning and continuity of care throughout recovery.

Coordinated Care Model
Our integrated care model allows patients to receive coordinated evaluation and rehabilitation within one clinic, helping reduce the need for multiple outside referrals.
Providers work across specialties to develop individualized recovery plans tailored to each patient’s symptoms, functional goals, and return-to-activity needs.
CCC communicates evaluation findings, treatment recommendations, and recovery progress with referring providers throughout care when appropriate.
